Since relocating from Maryland, Carolyn has begun to host tele-medicine sessions for her existing patients. Eligibility for these sessions does depend on your individual insurance and is currently offered for patients who are 18 years old and over. Please contact Megan Blankley (firstname.lastname@example.org) and she can quickly determine if you would be eligible to have a tele-medicine session based on your insurance.

THANK YOU FOR YOUR INTEREST IN TELE-HEALTH!
We are excited to offer a limited roll out of tele-health services this July. If things go well, we hope to offer additional appointment availability moving forward. Tele-health is simply a way to have a video chat session with your provider using a secure platform. This will allow you to have your appointment from home, work or wherever you are. The general format of your appointment will be exactly the same as it usually is, it will just be conducted virtually rather than in person.
Your co-pay will be the same and there is no additional fee for using this service.
You will need to complete additional consent forms and pay your co-pay prior to your tele-health appointment.
This service is not for emergency sessions and is for routine follow up only. If you are having suicidal thoughts or self-injurious behavior, tele-health is not an appropriate option, you should be seen in person for evaluation by another provider in the office.
Your prescriptions will be sent electronically and paper prescriptions will not be provided using tele-health.

USING THE PLATFORM
The tele-health platform is extremely easy to use. You can start your session from any internet connected PC as well as from your android or apple phone. Click here for general information about the platform: https://doxy.me/patients
Once you have an appointment scheduled, use the following link to “check-in” to the personalized waiting room of Carolyn Clark prior to your scheduled appointment start time:
You may want to use the link now to familiarize yourself with the set-up of the platform. You can check in at any point to practice using the website. You will need to give the site access to your microphone and camera, and it is recommended that you have this in place prior to your appointment so there is no technical hang up delaying the start of your appointment. You will be emailed this link again the morning of your appointment as a reminder. You may also consider saving the link to your browser now so you can easily check in prior to your scheduled appointment.
POLICIES AND PROCEDURES:
You will need to check in and be waiting in the virtual waiting room at least 5 minutes prior to the start of your appointment. Once you are checked in, your provider will start your session at your scheduled appointment time. If your provider is running late, you will be messaged using the secure chat messaging system on the doxy.me platform. If you are more than 10 minutes late checking into the waiting room, your appointment will be cancelled and you will be assessed a no-show fee of $95. If you cannot make your appointment for any reason, you will need to notify Ms. Clark via email at least 24 hours prior to your appointment at email@example.com. Cancellations made less than 24 hours prior to your scheduled appointment time will also be subject to the $95 fee.
